MOU Form

This MOU Form page is the first page
displayed after you access ASAP-MOU. The information you enter in the form's 24
boxes will be used to create the MOU Word document. Please enter/select
information for each question.

When you have completely filled out the
MOU Form, click the
button
at the bottom of the page. You will be shown an online draft of your MOU, which
you can then download and save as a Word document.

To erase any information you have entered
in the MOU Form and create a new MOU, click the link "New MOU Document" on the left side of the page.

To print any of the pages in ASAP-MOU
(for instance, if you want to keep easy track of what you put in the form),
click the Print button on your browser.
Entering Information in the MOU Form
The MOU Form has two different ways of
entering the required information: text boxes and drop-down list boxes. A text
box is a blank box , while a
drop-down list box is identifiable by the arrow .
To enter information in a text box:
1. Click the text box to position the
cursor inside it.
2. Type the information in the box as you
want it to appear in the printed MOU document
(for example, if you type "amer. airlines" , that is how it will appear in the
MOU).
To enter information in a drop-down list
box:
1. Click the arrow.

2. Click your selection from the drop-down
list.

3. Your selection will appear in the box.

Note:
Applicants may make minor modifications to the Word document generated by this
template, if there are special considerations that are not covered by the
template (e.g. multiple labor organizations), provided such changes do not
entail proposed modifications to FAA ASAP policy. The ASAP MOU is normally
based on agreement between three parties signified by each party signing the
MOU - the company, the FAA, and the employee labor organization Their agreement
is signified by each party signing the MOU's signature page. Not all employee
groups are represented by a labor organization. In these cases, the MOU will be
signed by two parties - the company and the FAA. Additionally, some employee
groups may have labor representation for some, but not all positions within a
group. One possible solution for this situation is to prepare two MOUs - one
for the employees that are represented by a labor organization, containing
three signatures, and one for employees who are not represented by a labor
organization, containing two signatures. Alternatively, the applicant may edit
the Word document generated by this template, to address the desired
arrangement in a single MOU. For additional assistance with this issue, feel

|
Type in the kind of operations conducted by this certificate holder (e.g."scheduled passenger service", "non-scheduled passenger service", "cargo operation", "aircraft maintenance and repair" etc.).

| 5. List Major Continents or Countries of Operation
|
Type in the areas of operation this certificate holder operates in (e.g. North and South America, Europe, etc.).

| 6. Air Carrier or Repair Station
|
Select type of business conducted (e.g. air carrier or repair station). If part 121 was selected in intem 3, "air carrier" should be selected here. If part 145 was selected in item 3, "repair station" should be selected here.

| 7. Flight-Based or Ground-Based MOU
|
Select whether this MOU is based on a flight-oriented position, such as a crewmember, or a ground-oriented position, such as a mechanic or dispatcher.

|
Type in the number of aircraft or repair stations operated by this certificate holder, as applicable.

| 9. Number of Employees Belonging to This Employee Group
|
Type in the approximate number of employees within the employee group covered by this MOU.

| 10. Employee Group Type
|
Select the type of position identified with this employee group (e.g. pilot, flight crewmember, flight attendant, flight follower, dispatcher, or mechanic).

| 11. Employees Represented by Labor Association
|
Select "Yes" or "No" to identify whether or not any or all of the employee group covered by this MOU is represented by a labor association. (See note above if group has mixed labor representation.)

| 12. Name of Employee Labor Association, Group or "N/A"
|
If a labor association represents this employee group, type in its name here, or enter "N/A" if a labor association does not represent this employee group.

| 13. Employee Labor Association Abbreviation, Group Abbreviation or "N/A"
|
If a labor association represents the employee group covered by this MOU, type in the first letter of each word in the labor association's name or use another commonly recognized abbreviation for the labor association's name, or enter "N/A" if a labor association does not represent this employee group.

| 14. Demonstration or Continuing Program MOU
|
Select "Demonstration" or "Continuing Program" based on the program type. Initial MOU submissions and any extension of them are "Demonstration Programs".

| 15. Number of Months for Demonstration Program ("N/A" if Continuing Program)
|
Type in a number from 12 to 18. 18 months is the standard duration of a Demonstration Program MOU. This field is used for Demonstration programs only. Enter N/A if this is a Continuing Program MOU.

| 16. ASAP Form Number or "form number not applicable"
|
If the certificate holder has developed an ASAP form and assigned a form number to it, enter that form number here. Otherwise, enter "form number not applicable".

| 17. Time Limit Statement
|
Select the appropriate description of the end of the employee's workday. This statement is used in determining timeliness of report submission, when appropriate. The choices are "the end of flight sequence for day of occurrence" used with flight-based MOUs and "the end of the duty shift" used with ground-based MOUs.

| 18. Name of Publication or Newsletter
|
Type in the name of the certificate holder's publication that will provide ASAP-related information to its employees.
|
| 19. Frequency of Publication
|
Select how often the above publication is printed or updated and made available to the certificate holder's employees. The choices are weekly, monthly, quarterly, semi-annually, or annually.

| 20. Name of Company Manual Containing ASAP Information
|
Type in the name of the certificate holder's manual containing information about ASAP (e.g. Flight Operations Manual, General Maintenance Manual, Safety Manual, etc.).

| 21. Means of Submitting ASAP Report
|
Type in the available methods of submitting ASAP reports (e.g. fax, company website, company email, etc.).

| 22. Company Position Title (signature page)
|
Type in the position title of the company officer that will sign the ASAP MOU on behalf of the certificate holder (e.g. Chief Executive Officer, Vice President of Operations, Director of Safety, etc.).

|
Type in the position title of the labor representative that will sign the ASAP MOU on behalf of the labor organization representing the covered employee group (e.g. President).
