How do I submit a supplemental record?
A.
To do a supplemental without having to retype everything, simply fill out the new record using the same control number of the original record. We will see both records here and update the initial record with the new information. Please place the words "supplemental information" in the problem description field to alleviate any confusion. Q.
How do I submit photographs relating to a SDR?
A.
If you have photographs that you feel better show the SDR that you are submitting, please send an email to

with the photograph attached along with the Unique Control Number or date of submission and aircraft ID.
Q.
How do I submit a "Component Only" SDR?
A.
For 145 Repair Stations that may be working just components and not see the actual aircraft: You can submit component records, simply fill out all the information about the defective part of the component, in section 5 (Specific Part or Structure Causing Difficulty) and give the component information in section 6 (Component/Assembly That Includes Defective Part).
For example:
You can submit the record without the aircraft, engine, or propeller information. If you have the specific part that failed on the component (i.e. piston on a mlg actuator), put the piston information in Section 5 (Specific Part or Structure Causing Difficulty) and the actuator information in Section 6 (Component/Assembly That Includes Defective Part).
If you haven't that detailed information, place the actuator information in the Part Causing Problem section and submit a supplemental when the cause of the malfunction or defect of the actuator is discovered. To submit a supplemental, use the control number of the original record and send the cause of the failure.
